What Fatty Liver Disease Means

How Fatty Liver Disease Can Progress
Fatty liver disease exists along a spectrum.
Steatosis
At the earliest stage, excess fat is present but there may be little inflammation or liver-cell injury. This is sometimes called simple steatosis. Many people remain at this stage, particularly when metabolic risks are addressed.
Steatohepatitis
In MASH, fat accumulation is accompanied by inflammation and injury to liver cells. This inflammatory state increases the chance that scar tissue will form. MASH cannot always be distinguished from simple steatosis using routine liver enzymes or a standard ultrasound alone.

Blood Tests

Managing blood sugar, blood pressure, triglycerides, and cholesterol may reduce the forces driving liver injury while also lowering cardiovascular risk. Medication should not be withheld simply because it is processed by the liver; the relevant question is whether it is appropriate and safe for that individual.
Reduce Alcohol Exposure
Alcohol can add metabolic stress and liver injury to an organ that already contains excess fat. People with MASH, fibrosis, cirrhosis, or abnormal liver tests may be advised to avoid alcohol. Others should discuss their intake honestly with a clinician because risk depends on both the quantity and the condition of the liver.
Review Medications and Supplements
An up-to-date list should include prescriptions, over-the-counter medicines, herbal products, vitamins, bodybuilding products, and concentrated extracts. Some supplements marketed for liver cleansing have been associated with liver injury, and product labels do not always provide a complete picture of safety or purity.
How Fatty Liver Disease Is Treated
Treatment begins by identifying the cause, estimating fibrosis risk, and addressing metabolic and cardiovascular health. For many people with early MASLD, the main approach remains sustainable nutrition, physical activity, weight management when appropriate, and treatment of diabetes, high blood pressure, and lipid abnormalities.
People with alcohol-associated disease need a plan focused on reducing or stopping alcohol safely. Medication-related disease may require adjustment of the suspected drug under medical supervision. Advanced fibrosis or cirrhosis usually requires specialist management and monitoring for complications.
Prescription Treatment for Selected Patients
The treatment landscape has changed. In 2024, the FDA approved resmetirom, sold as Rezdiffra, alongside diet and exercise for adults with noncirrhotic MASH and moderate-to-advanced fibrosis. The approval used the accelerated approval pathway, meaning continued approval may depend on confirmation of longer-term clinical benefit.
In 2025, the FDA approved semaglutide, sold as Wegovy, for adults with noncirrhotic MASH and moderate-to-advanced fibrosis. This indication was also granted through accelerated approval, with an ongoing trial assessing whether improvements in inflammation and fibrosis translate into fewer major liver-related events.
These medicines are not intended as universal treatment for anyone with liver fat. Patient selection depends on fibrosis stage, other health conditions, medication interactions, contraindications, and the balance of potential benefits and harms. Lifestyle and cardiometabolic care remain part of treatment even when a medication is prescribed.
What Does Not Prevent Fatty Liver Disease?
Several popular strategies sound liver-focused but do not address the main causes of disease.
Detox Drinks and Cleanses
A cleanse does not extract stored fat from liver cells or reverse fibrosis within a few days. The liver already processes chemicals continuously. Prevention depends on reducing ongoing metabolic and toxic stress, not temporarily replacing meals with juices or teas.
Extreme Fasting
Severe or prolonged food restriction may produce rapid weight loss and inadequate nutrition. Both can worsen liver health in some circumstances. Gradual change is safer and more likely to be maintained.
Relying Only on Liver Enzymes
ALT and AST are useful but incomplete. A person may have meaningful fibrosis with liver-enzyme levels that are not dramatically elevated. Risk assessment should consider diabetes, age, platelet count, imaging, fibrosis scores, and other clinical information.
Assuming the Condition Only Affects People With Obesity
Obesity is an important risk factor, but it is not required. Genetics, visceral fat, diabetes, cholesterol abnormalities, alcohol, medications, and uncommon diseases can contribute in people across a range of body sizes.
